Network Contract DES

We provide this service.

It requires practices to work collaboratively to best support the needs of their local practice population.  We are working with other local practices under the banner of Gorton and Levenshulme Primary Care Network (PCN).

The main service currently provided by our PCN is evening and weekend appointments.  For full details see Evening and Weekend Appointments.

Learning Disabilities Health Check Scheme

We provide this service.

We will keep a register of all patient aged 14 and over who are known to the local authority social services department as having a learning disability, and offer and provide an annual health check tailored to their needs.

Violent Patients Scheme

We do not provide this service.

Where patients have been removed from a practice list with immediate effect due to an act or threat of violence or abuse, they are registered with the violent patient scheme.

NHS England is responsible for ensuring that all patients can access a GP and they run the Special Allocation Scheme which will assign such patients to an appropriate practice.

Seasonal Influenza Vaccination

We provide this service.

We will identify and invite eligible patients to receive an appropriate vaccination against ‘flu during the campaign season which is usually from September to March.  The eligibility criteria and vaccinations to be used are decided nationally and not by the practice.

We do not provide this service.

Travel Vaccinations

If you are going on holiday then you should always check if you need any vaccinations for your chosen destination.  NHS Scotland provide a great website that will give you advice about where you are travelling to, including what vaccinations are required and recommended. https://www.fitfortravel.nhs.uk/home

If you do need any vaccinations, then it is important to make an appointment as early as possible. Some vaccination courses require more than one injection and there has to be a gap between them. Some countries also require you to have had your vaccines a certain amount of time before you arrive there. We recommend you get in touch at least 4 weeks before you travel.

Our reception team will ask you questions at the time of booking so they can inform the nurse of you travel appointment. The nurse will obtain the correct vaccinations in time for your appointment.

Please be aware that the NHS only provides some vaccinations free of charge for travel purposes. If you need any vaccinations that are not available free from the NHS then you will have to pay for these privately. We ask for payment to be made before the appointment.

If you have been advised by the surgery to complete a review of your smoking on a regular basis, you will need to complete our Smoking Review Form.

Primary Care Mental Health

If you have been advised by the surgery to submit a mental health review, you will need to complete our Mental Health Review Form.

Baby Clinics

We hold a baby clinic each Tuesday between 2:00pm and 5:00pm. The clinic is run by a doctor and a practice nurse.
